Fire Investigation Reports or Fire Code Inspection Records

To obtain a fire investigation report or fire code inspection record, please use the Tacoma Public Records Center.

Request a Patient Record or Medical Report

To request patient records or medical reports you will need to do the following. 

Step 1: If you are the patient: 

• You need a signed Patient Request for Access Form.
• Driver's license/photo identification (a copy is acceptable for faxed or emailed requests).

Step 2: If you are not the patient: 

• You need a signed Patient Request for Access Form.
• A copy of the document/s granting legal authority to the representative who signed, for example, a birth certificate, or a designation of legal guardianship, or a medical power of attorney.
• The requestor's driver's license/photo identification (a copy is acceptable for faxed or emailed requests).
